一、境内申请企业须提供如下材料：
（1）《对外劳务合作项目确认申请表》
（2）《对外劳务合作项目情况表》
（3）申请企业取得的《中华人民共和国对外劳务合作（外派劳务）经营资格证书》（复印件）
二、请境内申请企业联系对口英方雇主单位提供以下材料：
（1）英方雇主单位公司出具在英正式注册运营材料复印件。
（1）PhotocopyofRegistrationCertificateoftheBritishEmployer,issuedbyUKCompanyHouse.
（2）请英方雇主单位出具雇佣外籍劳务资质的证书复印件。
（2）PhotocopyofvalidSponsorshipLicenseoftheBritishEmployer,issuedbyUKHomeOffice.
（3）请英方雇主单位出具雇主公司情况介绍材料，以及联系人和详细联系方式。
(3)IntroductiontotheBritishemployeronaCompany-Title-Paperwithacontactpersonandcontactdetails,providedandsignedbythelegalrepresentativeoftheBritishEmployer.
（4）请英方雇主单位出具律师函，确认以下内容：该雇主单位与你公司签订开展劳务输入合同；并委托你公司协助其开展中国对英国劳务输出业务；聘用人员由英方雇主单位直接合法雇佣。
(4)TheBritishEmployershallpresentaletterpreparedandsignedbyapracticinglawyerintheU.K.,certifyingthefollowing,a）theBritishemployerhassignedacontractwiththeapplyingChinesecompanyregardingthesupplyoflaborfromChinatotheU.K.;b)theBritishEmployerhasentrustedtheapplyingChinesecompanytoconductlaborimport/exportinaccordancewiththecontract;c)thelaborsuppliedfromChinawillberecruitedbytheBritishEmployerinaccordancewiththecontractandtheU.Klaws.
